Abstract Background The evidence for the treatment of midthoracic fractures in elderly patients is weak. aim this study was to evaluate midterm results after posterior stabilization unstable elderly. Methods Retrospectively, all aged ≥65 suffering from an acute fracture treated with were included. Trauma mechanism, ASA score, concomitant injuries, ODI score and radiographic loss reduction evaluated. Posterior strategy divided into short-segmental long-segmental stabilization. Results Fifty-nine (76.9 ± 6.3 years; 51% female) caused by a low-energy trauma mechanism 22 (35.6%). Twenty-one died during follow-up period Remaining ( n = 38) followed up mean 60 months.
